Garfield County Collaborative Management Program
The Collaborative Management Program (CMP) was created in 2004 with the passage of HB 04-1451 to promote and encourage local collaboration, aiming for improved services and better outcomes for the children, youth, and families we serve.
CMP Processes
Through these processes, the CMP can serve children, youth, and their families who seeking community resources and care planning, children and youth at risk of truancy, youth who are at risk of or are involved in the juvenile justice system, children and youth who are at risk of or involved with the child welfare system, and professional in need of brainstorming around community resources.
- Integrated Service and Support Teams (ISSTs) in the development of the plan include two or more system representatives who are present to assist a child, youth, or family in developing an integrated service plan directed by family needs, identifying goals, facilitating collaboration, and participating with partners.
- The Multi-System Prevention Program is a multisystem prevention program that must include two or more system representatives. These representatives establish a program that facilitates collaboration and addresses needs not currently provided within the community.
